What happens to the cooling water in a normally functioning nuclear plant?

does it all become radioactive eventually? i know there are sealed systems and then outer non sealed systems. But how is the radioactivity treated ? On land what do they do with it ? And in nuke submarines is it just discharged willy nilly ?
